Set and Setting: Why They Determine the Experience

Set and setting realistically influence the psychedelic experience, but in measurements, they yield to dosage. We check which elements of protocols have research.

The same substance at the same dose can give the same person an experience described as deeply meaningful once, and a panic episode another time. The difference has been explained for decades with two words: set and setting. This concept is not folklore of the counterculture, but an element of research protocols in centers like Johns Hopkins. It also has its own literature, which is more restrained than popular summaries. The largest meta-analysis to date has quantitatively verified this, and the result turned out differently than the popular version of this concept suggests. In this text, we separate three layers: what has been measured in this regard, what is purely a description of clinical practice from one center, and what Polish law says about the substances themselves. The text is educational and concerns research; it is not an instruction for using anything.

What do the words set and setting mean exactly?

Set is the totality of internal conditions of a person at the moment of substance intake: mood, expectations, intention, fears, beliefs about one’s ability to cope with a difficult state, and what has happened in the preceding weeks. Setting is the external environment in three layers: physical, social, and relational. It concerns the room, but also who is in it and what relationship connects these people.

The term was popularized by Timothy Leary in the 1960s, although the observation itself is older and stems from psychiatry working with altered states of consciousness. We have not been able to find publications from that period in available literature databases, so historical attributions circulate in popular texts second-hand, and we treat them as such here. We further rely solely on works that can be opened and verified today.

It is worth noting that both concepts are descriptive categories, not variables measured on a single scale. This has serious methodological consequences. When a study says that something in the environment influenced the outcome, it usually measures one specific element: the presence of equipment, the level of anxiety before the session, or the assessment of the relationship with the facilitator. It does not measure setting as a whole, because such a measure does not exist.

Do set and setting matter more than dosage?

No. This is the most frequently repeated reinforcement of this concept and the easiest to verify. Studerus’s team collected data from 23 controlled experimental studies: 409 psilocybin administrations in 261 healthy volunteers. They checked 24 predictive variables, including age, gender, education, personality traits, previous experiences with substances, mental state before administration, experimental conditions, and dosage. Linear models were fitted for 15 variables describing the reaction.

The result is unequivocal and says something different than popular summaries. Dosage was clearly the strongest predictor for all measured reactions. Non-pharmacological variables had a significant contribution, but alongside dosage, not instead of it. Pleasant and mystical experiences were most strongly associated with a high score in the absorption trait, emotional arousal immediately before administration, and a low number of psychological problems in the preceding weeks. Unpleasant or anxious reactions were associated with high emotional reactivity, lower age, and study conditions using positron emission tomography (Studerus et al., PLoS ONE, 2012).

The last item is the best illustration of the entire concept because it shows it without metaphysics. A study in a scanner is simply a less comfortable environment than a calm room, and this is evident in the data. Therefore, the honest sound of the thesis is narrower than the title of this article: set and setting do indeed change the character of the experience, but do not replace pharmacology.

Which elements of the environment have measurement, and which are descriptions of practice?

Separating these two layers is more important in this topic than elsewhere because descriptions of offices circulate in popular materials as a list of requirements, although they were created as a report from the practice of one center. The table below shows what each of the repeated elements stands on.

The row about music requires commentary because it is most often misrepresented. The survey included ten people with extensive experience in conducting sessions and yielded 24 different recommendations for the peak phase and 24 for the preceding phase. Analysis of 22 of them showed that peak phase music has a regular, predictable phrase structure, a sense of continuous buildup, and less clarity of timbre than earlier music. However, the authors state directly that there are currently no real guidelines for selection or standardization between centers (Barrett et al., Frontiers in Psychology, 2017).

Why does the presence of a second person change the experience?

Safety guidelines from 2008 treat this as a safeguard, not as an addition. They recommend the presence of at least two monitoring persons throughout the action of the substance, building relationships and trust before the session, and preparing the participant. They also indicate that the presence of both genders in the monitoring team was recommended. Monitoring persons are present but do not initiate conversation on their own (Johnson et al., Journal of Psychopharmacology, 2008).

The justification in these guidelines is very specific and worth reading literally. The most likely threat is overwhelming distress during the action of the substance, which can lead to dangerous behaviors, such as leaving the session location. Less frequent, but more serious, are prolonged psychotic states. The presence of a second person mitigates the first risk and shortens the response time for the second.

This is also a place where it is worth stating directly what such a safeguard does not replace. In research protocols, it is preceded by qualification excluding individuals with a psychotic diagnosis or with such a diagnosis in the family, as well as medical supervision during and after the session. A companion outside such a setup does not have either this knowledge or these tools, and the results obtained in conditions with full safeguards do not transfer to situations where they are absent. How do set and setting relate to the legal status in Poland?

All the literature described above was created in studies conducted under permits, in clinical centers, and under medical supervision. Outside such a setup, the legal picture looks different. Psilocybin is listed in the Polish catalog of psychotropic substances in group I-P under item 86, and psilocin under item 76. Possession of substances from this group is a crime under Article 62 of the Act on Counteracting Drug Addiction, while Article 62a allows for the discontinuation of proceedings for a small amount for personal use when the penalty would be purposeless.

It is also worth knowing one more distinction, as the corpus of texts on psychedelics regularly loses it. The catalog does not have an entry called hallucinogenic mushrooms. Mushrooms are covered by regulations only through the substances they contain, not as a separate category. This does not change the legal assessment of the act, but it does change which regulation can be invoked. They are also not covered by Article 63, which pertains to the cultivation of poppy, hemp, and coca plants.

The practical sense of this section is as follows: the concept of set and setting describes a psychological mechanism and does not create a space where anything becomes permissible.
