The difference thus exceeds double the price, for the same product and the same prescription. This article shows the recorded prices of three products, explains where such a discrepancy comes from, and goes through the rules that determine costs in practice: the requirement for a personal examination of the patient, the 90-day limit on prescriptions, and the procedure for exporting the drug abroad.

Two things are worth noting from this table. A higher THC content does not automatically mean a higher price per gram, as the most expensive in terms of conversion turned out to be a product of average potency. The second thing is availability: none of the three products was available in more than one quarter of pharmacies on the day of the reading. Why does the same product cost twice as much in two pharmacies?

Because no one regulates this price. Hemp flower is not a ready-made medicine with a fixed payment, but a pharmaceutical raw material for preparing prescription drugs, and the margin on such raw material is a matter for the pharmacy. This role was assigned to it by the act of July 7, 2017 (Dz.U. 2017 poz. 1458, in force since November 1, 2017), which added Article 33a to the Act on Counteracting Drug Addiction; the permission to allow such raw material to be traded is issued by the President of URPL for 5 years. In the price lists we checked on August 11, 2026, Cannabis flos products appeared exclusively with the full payment price, without the level of reimbursed payment.

In addition, there are three supply-side factors. Products come to Poland from foreign manufacturers, so the currency exchange rate translates into the wholesale price. The pharmacy orders flower based on specific demand, rather than keeping it in constant circulation, which raises the unit cost with low turnover. A short expiration date of batches can also be a reason for discounts: in some pharmacies, the same strains were offered cheaper on the day of the reading with a note about the approaching date.

The practical conclusion is uncomfortable but simple. Until you call or check the comparator, you do not know how much you will pay, and the difference on one package can amount to several hundred PLN. What does the path from the office to the pharmacy look like?

It starts with a personal visit, and this is a requirement of the regulation, not good practice. A prescription for a product from Annex No. 2 to the regulation on narcotic drugs is issued after a personal examination of the patient by the person issuing the prescription, and in this annex, under items 4 and 5, are precisely the flowers of cannabis other than fiber, extracts from them, and cannabis resin (Dz.U. 2025 poz. 1678, § 7 ust. 2a pkt 2, reading August 2026).

The exception is narrow and does not apply to commercial teleconsultations. The regulation exempts from personal examination only the physician providing services under the primary healthcare agreement, and only when continuing previously initiated treatment with that product. The announcement “prescription for flower online in 15 minutes” describes a service that the physician should not fulfill according to this regulation.

Furthermore, there are two quantitative limits. One prescription can cover a maximum of 90 days of use, and the doctor can issue up to three prescriptions for consecutive periods, totaling also up to 90 days (§ 7 ust. 1 and 2). Only one such product can be prescribed on a prescription for a narcotic substance, and no other medications can be added to it (§ 6 ust. 2). How to calculate the cost of a month of therapy?

You need two numbers, and you will only find one of them online. The first is the price per gram at a specific pharmacy, which is something between 24 and 80 PLN according to the reading from August 11, 2026. The second is the daily dose, and this is determined solely by the physician issuing the prescription and is written on that prescription. We do not provide any number of milligrams or grams here, as the dose depends on the diagnosis, tolerance, and other medications of the patient.

The calculation is then trivial: daily dose in grams times price per gram times thirty. It is worth doing it with the upper and lower ends of the price range, as with this spread, these two results can differ by double. Additionally, add the cost of the qualifying visit, the cost of subsequent control visits, and, if the doctor recommends vaporization, the one-time cost of the device.

Is it allowed to export medical marijuana abroad?

Yes, but with a document and after an application submitted in advance. The regulation of the Minister of Health from June 8, 2026, separates two cases. The document for intra-community delivery and acquisition, i.e., for travel within the EU, is issued by the appropriate provincial pharmaceutical inspector (§ 5 ust. 1). The document for import or export outside the EU is issued by the Chief Pharmaceutical Inspector (§ 6 ust. 1).

The deadlines are identical in both cases. The document is issued for a specified period, not exceeding 30 days, based on an application submitted no later than 15 days before the day of crossing the border (Dz.U. 2026 poz. 827, reading August 2026). Fifteen days is a deadline from the regulation, not a suggestion, so a trip planned with a week’s notice means traveling without the drug or without the document.

Note the mistake that often appears in guides about exporting drugs: for travel within the EU, the appropriate authority is the provincial pharmaceutical inspector, not the Chief Inspectorate. A patient directed to the wrong address loses those days that the regulation requires. An exception where the evidence is strong is drug-resistant epilepsy. Cannabidiol under the name Epidyolex received marketing authorization in the European Union on September 19, 2019, and its registered indication is as an adjunctive treatment for seizures in Lennox-Gastaut syndrome and Dravet syndrome in patients from 2 years of age, only in combination with clobazam (European Medicines Agency).
