The difference thus exceeds double the price, with the same product and identical prescription. This article shows the recorded prices of three preparations, explains the source of such variation, and goes through the rules that determine costs in practice: the requirement for a personal examination of the patient, the 90-day limit on prescriptions, and the procedure for exporting the drug abroad.

Two things are worth noting from this table. A higher THC content does not automatically mean a higher price per gram, as the most expensive product in terms of conversion turned out to be one of average potency. The second point is availability: none of the three products were available in more than one-quarter of pharmacies on the day of the reading. Why does the same product cost twice as much in two pharmacies?
Because no one regulates that price. Hemp flower is not a ready-made medicine with a set payment, but a pharmaceutical raw material for preparing prescription medicines, and the margin on such raw material is a matter for the pharmacy. This role was assigned to it by the law of July 7, 2017 (Dz.U. 2017 poz. 1458, in force since November 1, 2017), which added Article 33a to the Act on Counteracting Drug Addiction; the permission to allow such raw material to be marketed is issued by the President of URPL for 5 years. In the price lists we checked on August 11, 2026, Cannabis flos products were only available at full price, without any level of reimbursed payment.
Additionally, there are three supply-side factors. Products come to Poland from foreign manufacturers, so the currency exchange rate affects the wholesale price. The pharmacy orders the flower based on specific demand, rather than keeping it in constant circulation, which raises the unit cost due to low turnover. A short expiration date for batches can also lead to discounts: in some pharmacies, the same strains were offered at a lower price on the day of the reading with a note about the approaching date.
The practical conclusion is uncomfortable but simple. Until you call or check the comparison tool, you won't know how much you'll pay, and the difference on one package can amount to several hundred PLN. What does the journey from the doctor's office to the pharmacy look like?
It starts with a personal visit, and this is a requirement of the regulation, not just good practice. A prescription for a product from Annex No. 2 to the regulation on narcotic drugs is issued after a personal examination of the patient by the prescriber, and in this annex, items 4 and 5 refer specifically to the herb of cannabis other than hemp, extracts from it, and cannabis resin (Dz.U. 2025 poz. 1678, § 7 sec. 2a point 2, reading August 2026).
The exception is narrow and does not apply to commercial teleconsultations. The regulation exempts from personal examination only the doctor providing services under a primary health care agreement, and only when continuing previously initiated treatment with that preparation. Therefore, the announcement "prescription for flower online in 15 minutes" describes a service that the doctor should not fulfill according to this regulation.
There are also two quantitative limits. One prescription can cover a maximum of 90 days of use, and the doctor can issue up to three prescriptions for consecutive periods, also totaling up to 90 days (§ 7 sec. 1 and 2). How to calculate the cost of a month of therapy?
You need two numbers, and you will only find one of them online. The first is the price per gram at a specific pharmacy, which is something between 24 and 80 PLN according to the reading from August 11, 2026. The second is the daily dose, which is determined solely by the prescribing doctor and is noted on that prescription. We do not provide any number of milligrams or grams here, as the dose depends on the diagnosis, tolerance, and other medications of the patient.
The calculation is then straightforward: daily dose in grams times price per gram times thirty. It is worth calculating with both the upper and lower ends of the price range, as with this spread, these two results can differ by a factor of two. Is it allowed to export medical marijuana abroad?
Yes, but with documentation and after submitting an application in advance. The regulation of the Minister of Health from June 8, 2026, distinguishes two cases. The document for intra-Community supply and acquisition, that is, for travel within the Union, is issued by the appropriate provincial pharmaceutical inspector (§ 5 sec. 1). The document for import or export outside the Union is issued by the Chief Pharmaceutical Inspector (§ 6 sec. 1).
The deadlines are identical in both cases. The document is issued for a specified period, not exceeding 30 days, based on an application submitted no later than 15 days before the day of crossing the border (Dz.U. 2026 poz. 827, reading August 2026). Fifteen days is the deadline from the regulation, not a suggestion, so a trip planned a week in advance means traveling without medication or without documentation.
Note the common mistake in guides about exporting medication: for travel within the Union, the appropriate authority is the provincial pharmaceutical inspector, not the Chief Inspectorate. A patient directed to the wrong address loses those days that the regulation requires. What do studies say about the effectiveness of cannabis in pain?
Less than the popular narrative suggests. The 2026 update of the Cochrane review included 21 randomized and double-blind studies, totaling 2187 participants with chronic neuropathic pain. For products with a THC predominance, the authors found no clear evidence of an effect in terms of pain relief of at least 50%, with the certainty of evidence rated as very low. For balanced THC and CBD products and for CBD predominant products, the conclusion is similar (Ateş i wsp., Cochrane Database of Systematic Reviews, 2026).
The authors also noted something that weighs as much in a conversation with a doctor as effectiveness. THC predominant products may increase the frequency of adverse effects from the nervous system, and balanced products may increase the percentage of patients discontinuing treatment due to adverse effects. Both conclusions were accompanied by low certainty of evidence, so these are not numbers to remember, but rather a direction.
An exception where the evidence is strong is treatment-resistant epilepsy. Cannabidiol under the name Epidyolex received marketing authorization in the European Union on September 19, 2019, and its registered indication is as an adjunctive treatment for seizures in Lennox-Gastaut syndrome and Dravet syndrome in patients from 2 years of age, exclusively in combination with clobazam (European Medicines Agency). This is a separate medication and a separate pathway, not cannabis from the pharmacy.
